 Sacramento Chapter of CMSA and Senior Care Solutions present:

CEU Mini – Conference: Part 1: Perspectives on Complex Case Management, presented by Catherine Geraty – Hoag, MSW, LCSW. Part 2: Identifying & Addressing the Cause of Agitation, presented by Dr. Elizabeth Landsverk.

Upon Completion of this program, the health care worker will be able to identify the Learning Objectives:

Part 1: 1. Identify barriers to discharge for patients with complex and often chronic needs related to psychosocial, behavioral/mental health issues and financial insecurity.

  1. Strategies employed to mitigate barriers to effect safe discharge of patient with positive outcomes for length of stay management and reduction in readmission.

Part 2: 1. Assess for pain in dementia residents.

  1. Identify interventions and medical treatment options to alleviate pain.
  2. Assess if treatment was effective and how to identify possible side effects.
  3. Utilize resources and best practices on pain management for dementia residents.
